Third Party / Volunteer Vaccination Attestation

All visitors working as a third party provider and all volunteers must complete a vaccination attestation for the 2021/2022 school year.

Visitors are expected to declare their vaccination status and will have to submit proof of vaccination. Hard copies will not be accepted. Anyone who does not provide this attestation and proof is not permitted on the school grounds.
